CLINICS
If you suffer from particular medical conditions such as diabetes, asthma, chronic heart disease etc, you will receive letters requesting you attend the surgery for a medical review with our specialist nurse. It would be appreciated if you would book the appointment stipulated in the letter. We are obliged to send you three letters if we do not receive a response.
ANTENATAL CLINICS
Patients are seen by the midwife and the doctors by appointment. The midwife's antenatal clinic is held on a weekly basis.
BABY CLINIC
The health visitors hold weekly clinics by appointment.
ASTHMA CLINIC
A weekly asthma clinic is run by one of our practice nurses on Tuesday afternoons, by appointment only.
DIABETIC CLINIC
This is run by one of our practice nurses in conjunction with Dr Evans on a Thursday afternoon, by appointment only.
FAMILY PLANNING
All the doctors provide contraceptive care during surgery hours.
MINOR SURGERY
All the partners perform minor surgery which is carried out in special clinics. Please make an appointment in normal surgery time to discuss this.
SMOKING CESSATION
We have trained smoking cessation advisors in the practice who can offer support and advice on a one-to-one basis for those patients who wish to give up smoking. Please make an appointment at reception.
NEW PATIENTS
All new patients will be offered a health check with the practice nurse.
PATIENTS NOT SEEN WITHIN THREE YEARS
Patients aged between 16 and 75 who have not seen a GP within the last three years are entitled to request a consultation.
PATIENTS AGED 75 YEARS AND OVER
Patients aged 75 and over are entitled to a consultation with a doctor if not previously seen within the previous 12 months.
(All patients are, of course, entitled to a consultation with a GP at any time for a medical problem.)
PRIVATE MEDICAL SERVICES
Some services are not covered by the NHS and a charge will be made. This might include insurance forms, passport applications and various special examinations.
Fees for these services are displayed in reception.
CORONARY HEART DISEASE CLINICS
One of our practice nurses runs a weekly clinic where patients who are on medication for coronary heart disease are monitored.
CHILD IMMUNISATION CLINIC
We hold a weekly child immunisation clinic on Wednesday morning.
TRAVEL IMMUNISATION/VACCINATIONS
Travel advice and vaccination is available. Please make an appointment at least four to six weeks in advance of your holiday to ensure adequate cover.
The practice is a Yellow Fever Centre and can give Yellow Fever vaccinations to patients not registered at the practice. Some vaccinations may incur a fee and/or a private prescription if they are not covered by the NHS. Some of the vaccinations we can provide are detailed below:

If you require general advice on travel immunisation, please telephone the surgery and request a telephone call from the travel clinic nurse. If, however, you are travelling to more exotic locations, multi destinations or backpacking, we recommend that you contact MASTA on 09068 224100 or check out their website at www.masta-travel-health.com who provide an excellent service.
They can also deliver up-to-the-minute advice on which areas of the world require tablets for malaria protection and which type. Most can be bought over the counter from a chemist but some will require a private prescription from your GP (which will incur a charge per prescription per patient). (Note that the chemist will charge for the cost of the tablets and the dispensing cost.)
Any other medication solely requested for travel purposes, eg antibiotics in the case of traveller's diarrhoea, will also incur a private prescription charge.
There are plenty of good sources of information available on the internet and we would recommend the following:
INFLUENZA AND PNEUMONIA VACCINATION
Influenza vaccinations are particularly recommended for patients with heart, lung, liver or kidney disease, diabetes, residents of nursing and rest homes as well as all those aged 65 and over. It is also available to all those who are the main carer for an elderly or disabled person whose welfare may be at risk if the carer falls ill.
TRIAGE CLINIC
We have a daily triage clinic which is run by our practice nurses. Triage appointments involve assessment and examination and will result in one of the following outcomes:
*Advice and use of pharmacy-purchased products
*Issue of prescription and advice
*Advice and use of pharmacy products and routine appointment with a GP
*Review by a GP after a short wait or an offer of same-day appointment
Triage Clinic appointments are only available on the day.
BLOOD TEST APPOINTMENTS
Due to demand, blood test appointments will only be given for the following:
Warfarin
Methotrexate
Urgent requests from the GP (who will mark the blood test form accordingly)
Frail/ disabled patients (for whom transport may be a problem)
Patients who wish to make an appointment for a routine fasting blood test for cholesterol and glucose are advised to make an appointment with the Sylvan Clinic (01489 575098) as they are able to provide more early morning appointments allowing Jubilee Surgery to use our early appointments for more urgent cases.
All other blood tests will be referred to the local phlebotomy clinics. You can obtain a list of these clinics from the surgery reception.
