Statements of fitness for work

As of 6 April 2010 the Sickness Certificate has now been replaced by the Statement of Fitness for Work.  This means that doctors will be able to advise patients about how they might be supported to work as they recover, as opposed to only being able to sign them off sick.

You do not need a doctor's certificate for any illness lasting seven days or less. Your employer may, however, require you to complete a self-certificate form (SC2). You will need a doctor's certificate known as a Statement of Fitness for Work (Form Med3) from the eighth day of illness. The doctor cannot issue this form without seeing you.

Accidents and emergencies

In cases of emergency, patients should dial 999 or go directly to the Accident and Emergency Department at Queen Alexandra Hospital, Cosham.

The St Mary's NHS Treatment Centre, Milton Road, Portsmouth provides a Minor Injuries and Walk-In Centre from 8.00am - 9.15pm seven days a week, including Bank Holidays. They can be contacted on 0845 076 5551.

There are also Minor Injuries Units at Gosport War Memorial Hospital, Bury Road, Gosport ( Tel 02392 794753) 8.00 am - 9.00 pm and at The Royal, South Hants Hospital on St Mary's Road, Southampton.

The Bitterne Walk-in Centre on Commercial Road, Bitterne (Tel: 02380 426356) is open every weekday evening between 6.30 and 10.00pm. On weekends and bank holidays it is open between 8.30am and 10.00pm.

The Hampshire Dental Out of Hours Service is available from 5.30 - 9.30pm, Monday to Friday and 8.00am - 9.30pm at the weekends and on Bank holidays and listen to the recorded message. You can also contact NHS Direct on 0845 4647 for help and advice, 24 hours a day.

Disabled access:

The surgery is all on ground floor level thereby improving access for the disabled.

Out of hours

A duty doctor is on call for emergencies that cannot wait until the next working day. The responsibility for providing out of hours cover between 6.30pm - 8.00am belongs to the Hampshire Primary Care Trust. You may call them direct on 01962 718697 or telephone the surgery where an answering service will give you instructions. For advice you may also call NHS direct on 0845 4647.

This is an emergency service who will pass your request on to the Duty Doctor. This service cannot take any other sort of message from patients, such as making surgery appointments or dealing with requests for repeat prescriptions.

It is more appropriate in extreme emergencies (eg. severe chest pain, collapse) to dial 999 for an ambulance, or accidents to go to Accident & Emergency Dept at the Queen Alexandra Hospital, Portsmouth.

Eye casualties should attend Queen Alexandra Hospital but must phone 02392 286162 or 02392 286062 before going to the hospital. There is also a walk-in centre at Gosport War Memorial Treatment Centre (Tel no: 02392 524611).
