We provide primary care services to patients registered with us in Fareham, Titchfield, Whiteley and surrounding areas. Our patients are welcome at all three of our surgeries at Highlands (Fareham), Jubilee (Titchfield) and Whiteley.

NHS Services Provision
Meon Health Practice is a Primary Care Network (PCN) and is contracted to provide NHS services by Hampshire and Isle of Wight Integrated Care Board (ICB).
Hampshire and Isle of Wight Integrated Care Board
The Integrated Care Board (ICB) is the statutory organisation responsible for setting the strategic plan for the NHS in Hampshire and Isle of Wight to deliver its part of the health and care strategy. It allocates NHS resources through our four places and our transformation programmes to make sure the right health services are being delivered to people across the area.
